Our Approach
We believe that while technology is a powerful tool, the magic lies in human connection. People may not remember the name of a program, but they remember their coach—the person who listens, encourages, and guides them. That trusted bond is what inspires real, lasting change.
Here’s how we support our participants:
A Personal Health Coach: Every participant is paired with a board-certified health coach who builds a trusted, ongoing relationship. They provide guidance, help set achievable goals, and offer non-judgmental support for the daily work of behavior change.
A Dedicated Care Team: Our coaches work alongside a remote clinical team of nurse practitioners, social workers, and community health workers, ensuring a circle of support.
Simple, At-Home Medical Devices: We provide easy-to-use, cellular-enabled devices like blood pressure cuffs and glucometers. The readings are sent to our platform in real-time, allowing our team to offer timely support and identify potential issues early.
Seamless Coordination with Doctors: We work as an extension of each participant's existing team of physicians. We provide them with clear summary reports and ensure our work aligns with their care plan, all without requiring them to adopt new software or workflows.
