Managing Chronic Conditions

Many people join Starlight to help with long-term health needs like high blood pressure, diabetes, heart disease, or kidney disease. Your certified health coach checks in regularly, helps you set goals, and makes sure you have the right tools to stay on track. With steady support and 24/7 access to a nurse, you’ll never have to manage your health alone.

Support After a Health Event

If you’ve recently been in the hospital or had a major change in your health, we’ll step in right away. During this time, you’ll receive more frequent phone calls focused on your discharge instructions and recovery. These calls are completely complementary, designed to give you and your loved ones peace of mind and to help prevent setbacks.

Cardiovascular Prevention

We partner with cardiologists to help you stay ahead of heart disease. You’ll use easy at-home devices to track your blood pressure and weight, with readings shared directly with your cardiologist and care team. Your coach checks in regularly to support heart-healthy habits around food, exercise, stress, and sleep, while our nurses keep an eye on your results and step in when needed.

1. We'll reach out

After you’re referred or sign up, someone from our team will call to explain how Starlight works, answer your questions, and confirm your insurance coverage so there are no surprises about cost.

2. Meet your coach

You’ll be matched with a certified health coach who takes time to understand your lifestyle and health goals. Your first call is free — just a chance to get to know each other and see if the program feels like a good fit.

3. Start monitoring with smart, connected devices

After a quick telehealth visit, you’ll receive cellular-enabled devices (like a blood pressure cuff, scale, and/or glucose meter) that are automatically shared with your health coach and doctor.

4. Ongoing Support.

Your coach will be by your side to listen, guide, and encourage you. They will check in regularly to review your numbers and help you build healthy habits that last. They will always be just a phone call, text, or email away.

Covered by Insurance

Starlight is covered by Medicare, Medicaid, and most commercial insurance plans. We’ll confirm your coverage upfront and let you know about any out-of-pocket costs before you start.

No insurance? We offer affordable self-pay packages. And if you’re joining after a hospital stay, your first 30 days of support are complimentary.

A Health Coach is a trained professional who works with you to support and guide you in achieving better health. They are your partner in managing chronic conditions, improving your overall wellness, and helping you make healthier lifestyle choices. All our health coaches are board-certified by the National Board of Health and Wellness Coaching, the gold standard and most recognized certification in the field.

Your Health Coach will collaborate with you to create a personalized plan based on your goals and recommendations from your doctor. They provide ongoing support through regular check-ins, keeping you motivated and on track. Whether you're managing a condition or simply aiming for better health, your Health Coach is here to help you succeed.

Our program is covered by most major commercial insurance providers, as well as Medicare and Medicaid. Rest assured, we will verify your coverage and inform you about any potential out-of-pocket costs before any billing happens.

Your coach will check-in with you at least once a month by phone call. The exact schedule depends on what works best for you, and you’ll decide on the right frequency together. If you ever need extra support, you can message your Health Coach any time through secure email, text message or our Patient Portal.

Absolutely! We encourage it. Your health coach will help make sure your care plan aligns with your doctor’s recommendations. We also create a Health Report with your medical device readings and health goals. If you'd like, we can send it to your doctor securely by email or fax—either as a one-time request or on a regular schedule.

You could be eligible to receive one or more of the following devices, depending on your diagnosed conditions:

  • Glucometer and testing supplies: Anyone with a diagnosis of diabetes.
  • Blood pressure cuff: Anyone with diabetes, chronic hypertension or gestational hypertension.
  • Weight scale: Anyone with congestive heart failure.
